Questions

When should I send it?

Within 24 hours. After that people have moved on and corrections stop coming.

Should I include everything discussed?

No. Decisions, actions and open questions only. Discussion that led nowhere is noise.

Who should be copied?

Everyone who attended, plus anyone with an action. Nobody else.
